Recent Warren County Food Service Inspections

The City of Warren and the Pennsylvania Department of Agriculture have released the results of food service inspections conducted in March and April.

The following establishments were inspected in Warren and received one-year license renewals:

Subway, 71 Market St., no violations.

Salvation Army, 311 Beech St., one non-critical violation: food stored in boxes on floor in walk-in freezer.

Moose Club, 422 Pennsylvania Ave. W., one critical violation: uncovered food found in prep refrigerator; and two non-critical violations: all staff not wearing hair restraints in kitchen area; and kitchen floor needs cleaned.

Kwik-Fill, 2016 Pennsylvania Ave. E., no violations.

First Church of God, 1111 Madison Ave., no violations.

Kwik-Fill, 201 Hickory St., no violations.

Chiodo’s, 413 Pennsylvania Ave. W., two non-critical violations: non-food contact surfaces need cleaned; and kitchen floor needs cleaned under and around appliances.

Grumpy’s, 904 Pennsylvania Ave. W., no violations.

The Bullpen, 200 Falconer St., one non-critical violation: kitchen floor needs cleaned under and around appliances.

The following Warren establishments were inspected but their licenses were not immediately renewed because the inspector said the license fee had not been paid at the time of inspection:

Pizza Hut, 25 Pennsylvania Ave. E., two non-critical violations: all staff not wearing hair restraints in kitchen area; and kitchen floor and walls need cleaned.

McCulla’s Steakhouse, 2043 Pennsylvania Ave. E., one non-critical violation: kitchen floor and ventilation system need cleaned.

The following establishments were inspected and declared in compliance by the Department of Agriculture through April 29:

Blue Jay Inn, 4114 Blue Jay Creek Rd., Sheffield, no violations.

Conewango Valley Country club, 4961 Market St., four violations: expired buttermilk in walk-in cooler; soda lines and/or non-integral cold plate device installed within ice bins and in direct contact with ice use for consumer beverages; several areas in need of detailed cleaning – under utensil storage shelving, under all cooking equipment in kitchen area, interiors and exteriors of plastic light guards, and condenser unit framework and surrounding ceiling of walk-in cooler; and heavy buildup of dust on fan exhaust screen.

Wells Hog Wild BBQ & Grill, 386 Kinzua Rd., no violations.

Dairy Delite, 1 Kinzua Rd., Clarendon, four violations: wet wiping cloths in food prep areas not stored in sanitizer solution; thick buildup of dust on wall mounted air conditioner intake fins; congealed food residue on can opener blade; and bottom of side entrance exterior door rusted with two holes about one inch in diameter.

J’z Texas Hot, 3 Railroad St., two violations: lack of backflow prevention device on main water line; and heavy buildup of grease and carbon on ventilation hood system baffles.

Lindell’s Hatch Patch, 522 Hatch Run Rd., no violations.

Sheffield Area Medical, 511 S. Main St., Sheffield, one violation: temperature measuring device for ensuring proper equipment temperature not calibrated/functioning properly in oven.

Bob’s Discount Foods, 6 Main St., Box 237, Sugar Grove, one violation: no sign posted to remind food employees to wash their hands – one basin in three-basin sink must be designated as hand-wash sink – and paper towel dispenser empty at employee bathroom area.

Fox Mart, 1 Cable Hollow Rd., Russell, seven violations: bottle of cleaner in pizza area without correct labeling; bottles of medicine stored among food in sandwich area; packages of beef sticks being sold in a container without proper identification – breakfast sandwiches in freezer should be date-marked – and a sign indicating “ingredients available upon request” should be posted; no air gauge thermometer in deli meat cooler and retail meat display case; lack of backflow prevention device on hose under three-basin sink; handwash sink in sandwich area being used as a food preparation sink as evidenced by observed tomatoes in sink; and mops are not being hung to air dry.

New Beginnings Family Restaurant, 1 Main St., Sugar Grove, two violations: containers of food stored on floor in walk-in cooler; and water-stained/damaged ceiling tiles in wait station area – corner trim missing/broken between handwash sink and walk-in cooler making area no longer smooth and easily cleanable.

St. Francis of Assisi, 343 E. Main St., Youngsville, no violations.

Columbus Elementary, 100 W. Main St., RD 2, Columbus, one violation: deeply scored and worn cutting board.

Columbus Inn, 11 E. Main St., Columbus, one violation: two tube lights above pizza oven do not have end caps.

Sugar Grove Elementary School, 1 School St., Sugar Grove, two violations: pressure measuring device for high-temperature dish machine not calibrated/functioning properly; and self-service single use tableware not displayed in a manner so that only the handles are touched by students.

Warren Area Elementary Center, 349 E. Fifth Ave., two violations: top door seal for warming unit damaged/broken leaving food storage cabinet exposed to the outside (temperature OK); and two lights out in walk-in cooler.

Warren Area High School, 345 E. Fifth Ave., no violations.

Buccardo’s Gas, Restaurant and More, 173 Main St., Tidioute, no violations.

Tidioute Community Charter School, 241 Main St., Tidioute, three violations: potentially hazardous food held at 44 to 46 degrees in reach-in cooler, unit not to be used for those food until capable of keeping products at 41 degrees or below; pressure measuring device for hot water sanitizing in mechanical warewashing equipment not functioning; and buildup of dust on mesh filters of baking equipment ventilation hood.

Tidioute Trading Post, 15910 Route 62, Tidioute, three violations: processed deli meat stored with no covering on storage racks in display case; handle of ground beef serving scoop stored in direct contact with the food; and time used instead of temperature to control ready-to-eat potentially hazardous foods without written procedures or documentation.

Hog Haven, 2481 Matthews Run Rd., Youngsville, one violation: hole in ceiling near main entrance and some flaking/chipping drywall on ceiling in dining area.

Youngsville Elementary Middle School, 232 Second St., Youngsville, two violations: two deeply scored and worn cutting boards; and loose hanging, chipping/flaking paint on wall above food preparation sink.

Youngsville High School 227 College St., Youngsville, two violations: food residue buildup and scale buildup in high-temperature dish machine; and two well worn and scored cutting boards.
