The City of Warren and the Pennsylvania Department of Agriculture recently announced food inspections conducted in November.
The following establishments were inspected in the city and issued valid licenses through Nov. 30, 2013:
NY Style Deli, 338 Pennsylvania Ave. W., no violations;
Country Fair, 413 Pennsylvania Ave. E., no violations;
Kim's Cafe, 220 Pennsylvania Ave. W., no violations;
Wendy's, 60 Pennsylvania Ave. E., no violations;
Kelly's Pub, 408 Pennsylvania Ave. W., no violations.
Six-month inspections were conducted at the following:
Family Dollar, 1036 Pennsylvania Ave. W., no violations;
Elk's Club, Hickory and Fourth, one critical violation uncovered food found in refrigerator;
Fifth Avenue Cake Works, 211 1/2 Fifth Ave., one critical violation - Safeserv certification not completed;
V&J Pizza, 601 Pennsylvania Ave., E., one critical violation - cutting board needs cleaned, sanitized or replaced; and one non-critical violation - appliances need cleaned inside and out;
Snuffy's Cafe, 208 Hickory St., one non-critical violation - appliances need cleaned inside and out, rusted shelves in refrigerator need replaced.
The following were inspected by the Department of Agriculture since October 7:
Aldi #88, 3917 Market St. Ext., one violation - heavy frozen condensate buildup on the condenser, no food stored under the unit;
Allegheny Pitt Stop, 306 S. Main St., Sheffield, one violation - rodent activity present in the dry storage area as evident by 13 rodent droppings counted on two different shelves;
Andrea's Hillside Lounge, Route 6, P.O. Box 928, Sheffield, one violation - plate warming unit had food debris on the bottom of the holding pan;
Arby's #6007, 3930 Market St., two violations - buildup of frozen condensate on the ceiling and floor of walk-in freezer, ice machine has a buildup of black residue on the ice dispensing chamber;
Bair's Pizza Pros, 5 N. Main St., Russell, out of compliance with eight violations - raw pizza dough in the walk-in cooler stored open with no covering... other containers stored directly on top of the exposed pizza dough, multi-use containers containing raw pizza dough observed to have food residue and was not clean to sight and touch, one light fixture in the walk-in cooler is not shielded or shatterproof, inside door of walk-in cooler in severe disrepair... pressed board on the inside of the unit door is severely cracked/falling apart... door no longer has a smooth easily cleanable surface, fly-like insects too numerous to count throughout entire facility... no preventative measures in place to control insects, four ceiling tiles located above the microwave in the food prep area are missing/broken, drip line for the walk-in cooler's refrigeration unit is missing... the unit is dripping condensate onto the floor and wall behind... no food products stored below, residue buildup and small hairs on inside and outside of the containers used to store raw pizza dough in the walk-in cooler... one container located on the bottom of the stack does not have a cover on it and a dirty container is stored directly on top of it... one dead fly-like insect was found inside the top storage container, facility is not cleaning the walk-in cooler at a frequency to preclude accumulation of dirt and soil... black mold-like substance observed growing on the wall below the refrigeration units and condenser as well as on the roof of the unit... dead fly-like insects (too numerous to count) located on floor to the right of the walk-in cooler;
Clarendon Cafe, 7 S. Main St., Clarendon, out of compliance with 13 violations - two fluorescent light tubes are not shielded or shatterproof over the food prep area; non-food contact surfaces not cleaned at a frequency to preclude accumulation of dirt, soil, and grease include exterior of fryer, food cooking equipment, mop sink, ventilation cover... dried raw meat juice located in bottom of crisper of silver home-style refrigerator (no food products in crisper); paper towel dispenser empty at the handwash sink in the employee/patron restroom; accumulation of grease on hood exhaust located above fryer and center duct cover located near over; one open fuel cylinder with wick exposed stored on wire rack above food equipment; employee's open coffee cup was observed on cutting board, a food contact surface, in food prep area; floor in the food prep area does not appear to be a smooth, easily cleanable surface; thermometers for ensuring proper food temperatures are not calibrated, stem-type thermometer indicated 52 degrees in ice water; food facility has a reminder statement on the menu for foods that are/or could be served raw or undercooked to the consumer, however, does not clearly disclose which foods the reminder statement applies to; wet wiping cloths in food prep area not being stored in sanitizer solution; potentially hazardous ready-to-eat food prepared in the food facility and held for more than 48 hours is not being date marked; dried food and sauce buildup on interior/exterior of buckets used to shake cooked wings and sauce, dried food residue and hardened grease accumulation on interior of fryer, accumulation of grease and food residue on plastic container in which cooked meats are stored, reach-in refrigerator located nearest shelf storage area; dry food residue buildup on blade of can opener;
Panda China Buffet, 1650 Market St. Ext., C-1, out of compliance with 13 violations - person in charge does not have adequate knowledge of food safety as evidenced by this non-compliant inspection; ice located in the reach-in ice machine is being used as a food item in drinks; bulk dressing containers in the food prep area are not labeled with the common name of the food and are not unmistakably recognizable; cutlery knives being used in contact with various food without being properly cleaned and sanitized prior to use, the handles of the blades had a buildup of soil and food debris; observed various master cartons of food stored directly on the floor in the walk-in freezer and walk-in cooler rather than six inches off of the floor as required; a plastic commercial garbage can is being used as a container to hold raw frozen chicken in the walk-in freezer, the facility does not have the capacity to properly wash, rinse and sanitize this type of container; ice machine is in a state of disrepair, the exterior top of the ice bin is cracked, not smooth and easily cleanable, you can right into the interior from the exterior, the unit is located in far corner of food prep area and is not easily accessible for cleaning and maintenance; mechanical dishwash machine observed with buildup of filth and scale buildup and not cleaned before use and frequently throughout the day, as evident by excessive scale and filth buildup on the interior doors and all over the machine; chlorine chemical sanitizer residual was not detected in the final sanitizer rinse cycle of the low temperature sanitizing dishwasher and not 50 to 100 ppm as required; blade of the can opener has dried food residue and mold like substance; black mold like substance located on backsplash wall of the wait station; plumbing system not maintained in good repair, observed drain trap leaking in the kitchen/food prep area; soap was not available at the handwash sink in the kitchen area and wait station, the facility is using Green Detergent, a dish and glassware soap, an industrial manual dishwashing detergent, not intended for hand washing, a warning statement on the abel states avoid contact with eyes and skin; evidence of rodent activity located in the bottom storage area of the hot hold buffet table located nearest the kitchen, observed 12 rodent-like droppings that did not appear to be fresh, did not observe any other rodent activity;
Shaw House, 15 S. Main St., P.O. Box 814, Sheffield, out of compliance with 12 violations - dried food residue on the blade of the can opener; lights are not shielded or shatterproof over the three-basin sink in the bar area and one light fixture in the walk-in cooler; master container of celery being stored on bottom of two door reach-in cooler located at the end of the bar nearest entrance to walk-in cooler area where there is considerable condensate buildup; drip line in the two door reach-in cooler... shows evidence of staining down back wall of unit and condensate buildup on bottom of unit; drip line in the walk-in cooler is draining onto the floor of the unit, condensate is accumulating and running down the wall behind the evaporator unit and flowing onto the floor of the unit (no food products are directly stored under this area); unpackaged chicken wings in food prep area are being stored in large chest cooler on kitchen floor in direct contact with ice; ice machine located in the food prep area does not have a physical air gap in the drain line to prevent backflow, also pooling water is accumulating under the unit; ceiling tiles are missing in the back room dry food storage area and need replaced, gap between ceiling panels that are in place and ceiling above should not be used as a storage area; heavy grease buildup located on filters of the ventilation hood, also grease is dripping down the wall behind the fryers and building up on the floor and underneath the fryers; food facility inspection indicates evidence of rodent activity in dry food storage area, facility does have a pest control program in place, shelves and table containing small reach-in refrigeration unit needs to be cleaned, staff needs to routinely monitor traps and surrounding area for evidence of rodents; three food storage containers located in the food prep grill area are not labeled with the common name of the food; heavy dust/cobweb accumulation under the bar counter, above drink ice basin located nearest beer taps, drinking is uncovered; working container containing green chemical located on work table near the three-basin sink is not clearly labeled with a common name or identifiable name;
Subway #3429, 2901 Market St. Ext., one violation - the facility does not maintain the original food employee certificate on site to be displayed in a location conspicuous to the consumer;
Tops #0671, 9 Leather St., Sheffield, two violations - bags of ice stored directly on the floor of the walk-in freezer rather than six inches off of the floor as required; fan guards to the cooling fans in the walk-in cooler have an accumulation of dust and floor of the walk-in freezer has scattered debris/litter;
Towne Tavern, Main and Railroad streets, Clarendon, four violations - two fluorescent tube lights are not shielded or shatterproof over the fryer; food employee certificate expired; heavy dust accumulation on power strip located under the bar counter above the three basin sink, six wing sauce shakers for cooked chicken wings are not cleaned at least every four hours, grease accumulation on wall behind fryer; exposed insulation hanging over the three basin sink in the manual warewashing room;
Youngsville Elementary Middle School, 232 Second St., Youngsville, three violations - dust accumulation on the evaporation unit in the walk-in cooler; door for the walk-in cooler closest to serving line does not open from the inside; toilet room does not have a self-closing door;
Youngsville High School, 227 College St., Youngsville, three violations - mechanical warewashing equipment observed with buildup of scale and food residue on the inside upper arm bars and inside track of the service doors; heavy dust/soil accumulation on evaporator unit in walk-in cooler; lack of a backflow prevention device on the threaded hose bib in the warewash area.
