The board of directors of the Tidioute Area Health and Dental Center and Warren General Hospital has mutually agreed to terminate their leasing relationship at the Health Center office located at 115 Main Street in Tidioute, effective October 31, 2019.

All current patient records and charts will remain with the hospital.

If desired and requested, a patient’s records can be transferred to another medical clinician by calling Warren Medical Group Administrative Support at (814) 230-2600.

The Health Center will bring another health care provider into the Health Center’s premises and health care services will continue to be provided.

The Health Center and the Hospital are working together to facilitate this transition and to insure that the medical needs of the community are met.
